Treatment approaches

The right care for your cancer, not someone else's

Dr. Weiner's practice spans every category of localized and advanced prostate cancer treatment. The plan is built around your tumor biology, your goals, and the best evidence available today.

Low-risk

Active surveillance

For carefully selected men with low-risk disease, structured monitoring with PSA, MRI, and biopsy can safely defer treatment, sometimes indefinitely. Our protocol uses biomarkers and imaging to detect change early.

Localized

Robotic prostatectomy

Minimally invasive removal of the prostate using the da Vinci surgical platform. Nerve-sparing techniques are used whenever oncologically safe to preserve continence and sexual function.

Localized

HIFU focal therapy

For select men with single-focus, intermediate-risk disease, high-intensity focused ultrasound treats only the cancer and spares the rest of the prostate. Dr. Weiner has published peer-reviewed research on genomic biomarkers that help identify the right candidates for focal therapy.

Localized

Radiation coordination

External beam radiation and brachytherapy are excellent options for many patients. Dr. Weiner partners closely with Cedars-Sinai radiation oncologists so you can compare options side by side.

High-risk

Multimodal therapy

For high-risk and locally advanced cancer, the best outcomes often combine surgery or radiation with hormonal therapy and access to clinical trials. We design these plans together with medical oncology.

Advanced

Recurrent & metastatic disease

Salvage surgery, advanced imaging (PSMA PET), genetic testing, and clinical-trial access for biochemical recurrence, oligometastatic, and castration-resistant disease.

Clinical trials open at Cedars-Sinai

Research that may be a fit for you

Cedars-Sinai is one of the country's most active prostate cancer research centers. Below are three trials currently enrolling. If one of them looks relevant, mention it at your consultation or contact the study team directly.

Active surveillance

Cholesterol-lowering therapy and immune cells

Who it's for: Patients on active surveillance with a biopsy scheduled in the next 3 to 6 months.

This Phase 2 study tests whether a commonly used cholesterol-lowering medication (typically Vytorin) can help the body's own immune cells (CD8+ T cells) move into prostate tumors and attack them. Participation involves taking the medication, routine blood work, short questionnaires, and the standard-of-care biopsy you were already planning.

Pre-surgery

WALNUTS for POWER, a diet study before prostatectomy

Who it's for: Patients with confirmed prostate cancer (Gleason 7 or higher) planning a radical prostatectomy.

This Phase 2 study looks at whether eating two ounces of walnuts daily in the weeks before surgery slows prostate cancer growth. Researchers compare biopsy and surgical tissue to measure changes in the cancer cell proliferative index. The study does not change the timing or type of your surgery.

Surgical safety

TRIPHASE: protecting the ureters during prostatectomy

Who it's for: Patients planning a radical prostatectomy at Cedars-Sinai.

This Phase 2/3 study tests Nizaracianine Triflutate (ZW800-1), an IV medication that makes the ureters glow under a near-infrared camera. The goal is to help surgeons see and avoid the ureters during pelvic surgery, lowering the risk of injury. All participants in the current phase receive the active medication.

Final eligibility for any trial is determined by the study team after a brief screening conversation. General clinical trials inquiries: clinicaltrials@cshs.org.

What to expect

Your first visit

Most consultations last 45 to 60 minutes. We review your imaging, pathology, and prior testing together and discuss every reasonable option, the trade-offs, and what an evidence-based plan looks like for someone in your specific situation.

Before the visit

Our team gathers your records in advance and confirms your insurance. If you would like a second opinion only and do not plan to transfer your care, that is welcome.

During the visit

Expect a thorough discussion of your tumor's biology and risk, the role of additional testing if any, and a side-by-side comparison of treatment options. You will not be rushed into a decision.

After the visit

You receive a written summary of the discussion and recommended next steps. We coordinate any additional imaging, genetic testing, or sub-specialist visits before treatment is scheduled.

Patient stories

What patients say

Reviews shared by patients on Google and Yelp.

"About a year and a half ago I was diagnosed with an aggressive form of prostate cancer. I had to make an important decision about surgery, and which surgeon to use. Dr. Weiner was my choice. He does a robotic 'nerve sparing' procedure that resulted in me having a very good outcome. I am very happy with the professionalism and talent of Dr. Weiner as a urologist and as a surgeon."

FAQ

Common questions

Do I really need surgery if my prostate cancer is low-risk?

Often, no. Many low-risk prostate cancers are best managed with active surveillance, where we monitor the cancer over time with PSA, MRI, and periodic biopsy. Treatment is added only if there is meaningful evidence of progression.

Will I be able to maintain urinary control and sexual function after surgery?

This is an individual discussion. Recovery of urinary continence and erectile function depends on your age, your baseline function, your tumor's location, and how much of the nerve tissue can be preserved. We talk through realistic expectations for your specific case before any decision is made.

How do I decide between surgery and radiation?

Both are excellent options for localized prostate cancer with similar long-term cancer control in many cases. The right choice depends on your tumor characteristics, your other health conditions, and how you weigh the side-effect profiles. Dr. Weiner partners with radiation oncology so you can hear both sides honestly.

What insurance do you accept?

Plans accepted include Aetna, Aetna HMO, Anthem Blue Cross, Blue Cross Blue Shield, Blue Cross of California HMO, Blue Shield of California, Blue Shield of California HMO, Cigna, Humana, Humana HMO, Kaiser Permanente, Medicare, UnitedHealthcare, and UnitedHealthcare HMO. Our team will verify your benefits before your visit.

Do you see patients for second opinions?

Yes. Many of our patients come for a second opinion only. We provide a written summary of our recommendations that you can take back to your local team if you choose to stay with them.

Do you offer telehealth?

Yes. Telehealth visits are available for established patients in California and for select consultations. Some pre-operative and post-operative visits can also be done virtually.

What about clinical trials?

Cedars-Sinai is an active research center, and Dr. Weiner's lab partners with the broader prostate cancer trial portfolio. If a trial is a fit for your situation, we will discuss it openly.
